Dynamic Pathway to Independence: #Stage1

The Initial stage of the Dynamic Pathway to Independence is the outreach stage, providing on the street support. We deliver the right support, in the right way, at the right time, ensuring that members of our team are available at any time of the day.

Thanks to our digital support journey, we now have our inform and outcome star system, fully mobilised on tablets, which means that our specialists outreach support workers can now be on the street where they need to be at whatever time of day.

‏We have specialist support workers who are there to work with and build up a relationship with the most vulnerable in our community. Meeting outside and not in an office is essential in working together and to help rough sleepers gain the full support they need. Developing trust and communication with our residents is vital at this stage to ensure they have the correct support system in place.
